Clinical Social Worker II
Under the direction from the supervising social worker, the Clinical Social Worker II (CSW II) will work with individuals living with HIV (or affected by HIV) and families to provide low threshold/low barrier social work services within an ambulatory care setting. These services include, but are not limited to: psychosocial assessments, assisting patients in referrals to emergency and long-term housing stabilization, substance use counseling, mental health assessments, and appropriate referrals to community agencies for financial benefits, legal, and immigration resources.
