As a Per Diem Registered Nurse, Care Coordinator at Cedars-Sinai you will be scheduled to work a minimum of 4 shifts per month on weekends and 3-4 weeks straight training to start. Responsibilities include the following.

• You will serve as a clinical co-pilot-charting the best trajectory of each patient?s care by accommodating various needs, including medical, psycho-social, financial and practical.

• You will coordinate the care and service of the patient population from admission through discharge.
• You will assess and Plan, implement, monitor, and evaluate options and services to meet an individual's health needs to facilitate a safe and timely discharge to the next appropriate level of care.

Qualifications:

Education:

• BSN required

License/Certification:

• Current, unrestricted California RN license
• American Heart Association BLS
• Case Management certification preferred

Experience:

• A minimum of 3 years Case of Management experience preferred
• At least 2 years of acute care nursing experience required
• Knowledge of medical literature, research methodology, health care delivery systems and financial reimbursement required
