Your Role on the Team

Position Summary:

The Physical Therapist II provides a full range of physical therapy services including evaluation, treatment planning, therapeutic interventions, and discharge planning. • REHAB Performs and acts as a clinical resources for advanced therapy evaluations and treatments 15%
• REHAB Counsels and educates patients families, and care givers 15%
• REHAB Completes appropriate documentation (assessments, progress notes, discharge summaries, care plans, and regulatory and third party payer information) in a timely manner according to department standards 15%
• Provides a full range of physical therapy services including evaluation, treatment planning, therapeutic interventions, and discharge planning 60%
• Performs and acts as a clinical resources for advanced therapy evaluations and treatments. 10%
• PT Provides a full range of physical therapy services including evaluation, treatment planning, therapeutic interventions, and discharge planning 45%
• Counsels and educates patients families, and care givers. 10%
• Completes appropriate documentation (assessments, progress notes, discharge summaries, care plans, and regulatory and third party payer information) in a timely manner according to department standards Communicates effectively with the appropriate Medical Therapy Units (MTU) as required. 20%
• Communicates effectively with the appropriate Medical Therapy Units (MTU) as required 10%

Required Qualifications:

• Ability to maintain a work pace appropriate to the workload
  
• Must demonstrate customer service skills appropriate to the job
  
• Excellent written and verbal communication skills in English
  
• If assigned to work with infants and children aged 0 to 21, CCS paneling/approval will be required within one year of hire if qualified for CCS paneling requirement
  
• Minimum 5 years' experience as a licensed physical therapist
  
• Physical Therapy License or proof of eligibility from California Board of Physical Therapy
  
• Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
  

Licenses and Certifications:

• BLS (Required)
