Job Description

This program is part of a 1-year feasibility initiative funded by the Cambia Health Foundation designed to enhance health outcomes, independence, and safety for low-income older adults in assisted living facilities.

The goal of the program is to develop a sustainable model that trains Community Health Workers to provide tailored interventions addressing the physical, emotional, and social needs of assisted living residents. These interventions will include discussions about serious illness, setting personal goals, and identifying strengths and resources.

We are seeking applicants who have:

1. A deep connection to and understanding of low-income older adult communities.
2. The ability to listen to and engage with older adults with intact cognition and mild dementia.
3. Expertise in bilingual communication (Spanish and English). 

Job Responsibilities:

Participant Engagement

• Travel to 3 AL facilities in East Harlem and the Bronx.
• Plan and implement strategies to identify eligible assisted living residents.
• Meet 1:1 and small group settings: Conduct interviews, obtain informed consent, and facilitate pre- and post-session assessments.
• Track participant progress and provide follow-up support.

Program Implementation

• Deliver culturally tailored one-hour sessions that include educational content, discussions, and skill-building exercises.
• Address participant concerns related to independence, health, safety, and well-being.
• Assist participants in articulating their goals, values, and preferences during program sessions.
• Document participant interactions and prepare reports for program evaluation.

Collaboration and Communication

• Build positive relationships with residents, staff, and community partners.
• Communicate regularly with program supervisors and stakeholders to share progress and address challenges.
• Participate in weekly team meetings and supervision sessions.

Administrative Tasks

• Maintain accurate records of program activities, participant progress, and resource utilization.
• Develop and update a database of community resources tailored to older adults.

In compliance with NYC?s Pay Transparency Act, this position is $30.00 per an hour (16 hours per week). New York University considers factors such as (but not limited to) the specific grant funding and the terms of the research grant when extending an offer.

• High school diploma or GED preferred.
