This Staff Nurse will be responsible for:
- Coordination of care for medically complex patients and acting as liaison to multiple services including Nephrology, Cardiology, Surgery, Interventional Radiology and referring providers.
- Telephone triage, care management and coordination, post-surgical follow-up, reviewing laboratory studies, scheduling diagnostic studies and procedures. This role also is responsible for preparing and facilitating weekly patient care meetings.
- Patient education and staffing the ambulatory clinic.
- Monday - Friday, days, 8 hour shifts.
To qualify, you must have:
- A Bachelor's degree in Nursing.
- Minimum of 1 year of RN experience.
- Nephrology experience preferred (adult or pediatric), inpatient pediatric experience preferred.
