Department Description
The Healthy Aging Clinic is an interdisciplinary clinic that provides care to patients 65 years of age and older. This is a new clinic to UC Davis Health that will promote healthy aging through an integrated approach across the lifespan and all care settings. As the clinic grows, we plan to have the following services within our clinic: Geriatric Medicine, Neurology, Cognitive testing with Neuropsychology, Geriatric Psychiatrist, Family Caregiving Institute and several other patient services. The Healthy Aging Clinic will be a part of providing age-friendly care, Supporting family caregivers, Leveraging cutting-edge research, Integrating technology for independent living, Educating the next generation of clinicians through degree programs, fellowships and certifications.

Responsibilities
As an LCSW, your primary role will be to assist patients and families/caregivers in coping with the social/emotional issues and practical arrangements related to patients' illnesses. You will deliver age-appropriate social work care to patients and their caregivers in accordance with agency policy and procedure and state and federal regulations. You will serve as an integral member of the healthcare team providing assessments, coordination, treatment planning, information and referral to community resources and other social work services to meet the complex needs of patients and families in the hospital and clinic settings.
In this position, you will assist with both Healthy Aging Clinic patients and California Alzheimer's Disease Center patients. You will be the primary host for a Caregiver Support Group, and assist with patient and/or caregiver classes offered by the CADC.
You will meet patients' needs through clinical assessment, accurate and sound judgement, and maintaining professional communication. You may be asked to participate in special projects and collect data to improve the quality of services delivered to our patients.
Required Qualifications

- Minimum of three years recent related professional experience.
- solid psychosocial assessment ability.
- Skills to perform in-depth chart analysis to provide accurate information in a concise, informative manner.
- Experience in identifying and analyzing care/service needs for patients.
- Skills to comprehend and assess patient's grievances to quickly locate appropriate resource for assistance.
- Excellent clinical judgment and problem-solving skills.
- Knowledge of care/services provided in differing levels of care, i.e., ambulatory and institutional settings.
- Ability to effectively and efficiently handle demanding workload involving multiple tasks.
- Knowledge of biopsychosocial and behavioral health treatment plans.
- Knowledge of chronic and acute disease and how it influences patient and family functioning.
- Knowledge of legal requirements and guidelines for providing psychiatric services, including mental status exams and 5150 applications.
- Graduation from an accredited school of social work (MSW/LCSW) with a minimum of 3 years recent experience.
- Valid California Board of Behavioral Sciences Licensed Clinical Social Worker license.
- Behavioral health experience
Special Requirements
This position may be subject to a criminal background investigation, drug screen, Live Scan fingerprinting, medical evaluation clearance, and functional capacity assessment.
