JOB SUMMARY/ESSENTIAL JOB FUNCTIONS: Under the direction of an assigned supervisor this position will be responsible for coaching, motivating, and supporting participants in the Equitable Health in Neighborhoods (EHiN) Program. Duties include interviewing and interacting with participants to promote physical fitness and nutrition activities to improve health outcomes and manage or prevent chronic diseases related to obesity.
DUTIES AND RESPONSIBILITIES:
Conducts virtual and/or in-person follow-ups and provide education to support continued program engagement.
Collaborates with clinicians and participants to design individual programs as well as make referrals to necessary primary and behavioral health care and other community supports.
Educates participants on disease prevention and advocacy for improved health care.
Coordinates services with internal and external behavioral and mental health care providers.
Conducts virtual and/or in-person follow ups to promote continued engagement and retention.
Makes referrals to community supports.
Works with community organizations and rental offices to recruit program participants.
Collects and enter data into database.
Performs other duties as required.
