Position provides direct patient care and education for patients receiving treatments, examinations, surgeries, etc., in medical office according to established procedures.
This position will support the Limb Preservation Program. Â This role will be responsible for supporting the multidisciplinary, patient-oriented service that will care for patients with lower extremity wounds, which have a high re-admission rate, high complication rate associated with any interventions, and a long-term high mortality rate. Responsible for triaging calls and referrals to determine the care needed to treat and manage these types of patients. Â This support will include a coordination of care between ACCS/Podiatry, Vascular, and Plastic surgery and their team members to allow for a streamlined approached in order for these patients to be seen in a more formalized fashion. Â This coordination of care would include scheduling appropriate testing and organizing consecutive appointments among services based on patient need. Â In addition, they would be responsible for capturing data related to length of stay, readmissions, and number of amputations performed following the initiation of this program. Â
Primary Duties and Responsibilities
Utilizes the nursing process in assessing, planning, implementing, and evaluating a plan of patient care.
Provides patient care, which may include administering medication, performing procedures, and monitoring patient status in accordance with approved nursing practice.
Provides appropriate counsel and education to patients/family regarding disease process and symptom management.
Maintains medical records requiring appropriate and timely recording of patient information.
Consults with members of the health care team, adjusting nursing care processes as indicated to ensure optimal patient outcomes.
Assists the health care provider with examinations, procedures, prescription refills, and other processes related to direct patient care.
Maintains required skills and competencies and participates in educational programs, staff meetings, and in services such as CPR as required.
Other duties as assigned.
One year of related medical experience.
Working knowledge of general nursing theory and practice with ability to recognize and interpret symptoms/conditions and ability to function in new and emergent situations.
Graduate of an accredited nursing program.
Current state of Illinois and/or Missouri LPN or RN licensure as applicable by state of work location, in accordance with Compact Act; Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements).
$49,800.00 - $89,700.00 / AnnuallyThe salary range reflects base salaries paid for positions in a given job grade across the University. Individual rates within the range will be determined by factors including one's qualifications and performance, equity with others in the department, market rates for positions within the same grade and department budget.
