Reports to the Day Center Director and is responsible for providing ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual client needs; enhancing the quality of client management and satisfaction through the integrating and functions of case management, utilization review, and goal planning; having accountability for the care, coordination and goal planning of all clients; and performing other related duties as assigned.
The Fair Labor Standards Act status of exempt or non-exempt will depend on the incumbent's entry salary for this position.
