The Digital Health Nurse Specialist is a member of the Population Health Clinical Quality team and reports to the Assistant Director, Clinical Quality Performance. This position is a key member of the Digital Health team, enrolling and managing patients with chronic conditions through digital technology. This position also analyzes data to support quality improvement strategies and/or patient outreach. This position also serves as a clinical resource for the Population Health Quality Digital Health Specialists.
Key responsibilities include:
Supports Digital Health program with clinical chart review to determine patient eligibility for services; case management of patients with out of control diabetes and hypertension; and technical assistance with digital devices. Assists with clinical data management including establishing systems to acquire data and developing appropriate reporting structures to support quality reviews. Conducts health coaching and motivational interviewing for patients on a routine basis supporting the care plan set by the primary care provider. Performs timely and accurate reporting of quality data to department leadership, health plans, and outside regulatory agencies.
Seven (7) or more years of relevant experience OR a Bachelor's degree in nursing or other healthcare profession and three (3+) or more years of relevant experience; or a combination of comparable clinical education and experience.
Registered Nurse (RN) license issued by the State of California.
Experience must include case management for patients with out-of-control hypertension or diabetes.
Experience with digital health tools (e.g. virtual health coaching via texting or online platform, training patients on Bluetooth enabled medical devices like blood pressure cuff and weight scale).
Experience and proven success in QI standards, clinical chart review and abstraction, and regulatory requirements.
Master's degree in nursing or other healthcare profession.
Bilingual in English/Spanish.
Must be able to work various hours and locations based on business needs.
