Provides department support for the Continuum of Care Team to facilitate discharge planning and ensure appropriate throughput of patients.Â Works with Case Managers, Transitional Care Coordinator, and Social Workers to ensure discharge plans are communicated to patients and families during hospitalization and post discharge to ensure continuity and identify clinical barriers.Â Â Enables a positive patient experience through the discharge process and connection to resources as needed.Â Â Â Â Â Â
Minimum Education:â¢High School graduate or equivalent. â¢Completion of an accredited vocational nursing program.Minimum Experience/Knowledge:â¢Minimum 2-3 yearsâ clinical experience. â¢Typing 40-55 WPM. Experience with computer data entry. â¢Proficient in Microsoft Office Suite. â¢Good organizational skills. â¢Strong command of the English language. â¢Good customer service skills. â¢Ability to multitask and work effectively in a team environment.Required License/Certification:â¢Valid California Vocational Nursing license required. â¢Fire and Safety Certification. If no card upon hire, one must be obtained within 30 days of hire, and maintained by renewal before expiration date. (Required within LA City only)
