Under general direction of Care Management leadership, the Clinical Social Worker provides social work services consisting of discharge planning, continuing care services and clinical social work services; including crisis intervention, mandated reporting, information and referral coordination of health and mental health services with community providers. The Social Worker is responsible for education and advocacy of patients, families and/or significant others. Participates in related department functions, such as community and program education. The Clinical Social Worker is required to work in all acute medical units including NICU and burn.
Master's Degree from an accredited school of social work (MSW).
A minimum of two (2) years post MSW experience.
Must possess excellent interpersonal and communication skills.
Must be able to work within a team environment.
California LCSW license.
Demonstrated clinical training and/or work experience in the acute care, inpatient setting.
Must be able to work various hours and locations based on business needs.
