What will you be doing in this role? As a Social Worker for the Complex Care Medical Social Work function you are assigned to the Complex Care Coordination Team who focuses on the physiologically and socially complex case patients. This position serves as referral service-based team who provides supportive guidance, resource management, and service recovery functions. The duties for this role include:
Specialized Service focused on complex medical and psycho-social needs
A Case Mix including (but not limited to) a combination of supportive peer consultations and primary complex case interventions
Assists with problem solving with escalated issues and intervenes during crisis
Performs assigned functions independently, exercising critical thinking, and consistent clinical judgement formulated from evidence-based practice
Maintains quality, patient satisfaction, and preservation of patient rights and dignity
Participates in multidisciplinary case conferences, outlier meetings, and physician-patient/patient family conferences to promote quality patient care
Formulates, implements, and documents goal-directed transition of care plans based on patient/family's participation & needs
Identifies appropriate community resources, abuse referrals, Probate Conservatorship, and coordinate services for patients and families.
Masters Degree in Social Work required.
Licensed Clinical Social Worker (LCSW) preferred.
2 years' experience in an acute care hospital preferred.
