At the Long Island State Veterans Home, the Assistant Director of Social Work Services is an integral part of our interdisciplinary health care team. The incumbent will assist the Director of Social Work Services and assume the role of department head in the absence of the Director of Social Work Services. The incumbent is responsible for providing the most comprehensive service possible to be able to assess and identify psychosocial needs and develop criteria to assure residents and families in need of social work assistance obtain needed services
Specific duties include, but are not limited to:
Provide Social Work Services to residents 60 - 90 assigned residents, including long term treatment, planned short term treatment or crisis intervention according to individual needs.
Assist resident family members and/or resident advocates in mediating facility and community systems.
Participate/initiate interdisciplinary team meetings for resident care provision and discharge planning.
Engage residents and/or families in professional treatment relationships to assess the need for Social Work intervention.
Provide social work practice to assist resident and/or family to cope with the psychosocial issues connected with illness and or end of life.
Assures compliance with the NYS code, Federal and State Rules and Regulations, local ordinances, as well as departmental policies and procedures. Maintain required documentation as per department policies and procedures and in compliance with regulatory requirements.
Assist Director of Social Work Services in planning, coordinating, and implementing programs which provide social service functions and which promote the service mission of the Social Services Department in accordance with accepted social work practice.
Assist departmental staff in the facility on matters related to Social Services policies and procedures and overall resident care policies.
Assist with planning, evaluating and interpreting statistical, narrative, and analytical reports to reveal department's services rendered and ensure needs of the residents served are met.
Attends staff meetings, conferences and appropriate committee meetings.
Supervises staff and participates in teaching and demonstrating the profession of social service to academic personnel, researchers, medical students and professional staff.
Assist in developing, implementing, and ensuring a high quality assurance plan for the department including but not limited to, the review and monitoring of important aspects of resident care using objective criteria and standards of Social Service practice which focuses on the quality and appropriateness of services being rendered. In-services and regular staff meetings about current methods and modalities to keep social workers abreast as it affects delivery of social services.
Provide Social Work Services orientation to new employees as well as provide annual education to LISVH staff regarding Resident's Rights and Resident Abuse.
Assumes other responsibilities as they are assigned.
In the Absence of the Director of Social Work Services, specific duties include but not limited to:
Serves as a discharge planning coordinator and develops and implements appropriate procedures.
Responsible for supervising and monitoring all direct care services provided.
Oversight of Resident and Family Council.
Act as liaison to the handling and resolving of resident grievances and concerns.
Advocates on the rights of the resident.
Attends Corporate Compliance and QAPI meetings quarterly and as needed.
Directs Social Work Services activities.
Responsible for the coordination of resident Board of Election activities.
REQUIRED : Master's Degree in Social Work from an accredited school of Social Work. A NYS Social Work license. Four years of active patient/resident contact in long term care which includes at least two years of social services clinical supervisory experience in long term care.
PREFERRED : Experience working with MDS, discharge planning, electronic medical records, and ADL software. Working experience with the veteran population. LCSW and Supervision in Field Instruction (SIFI). Drug and Alcohol Certification.
The LISVH is a 24/7 skilled nursing facility. As such, all positions are subject to changes in pass days and shifts as necessary to meet the operating needs of the facility.
