UW Medical Center – Montlake has an exciting opportunity for a full-time (100% FTE, 40 hours per week), day shift REGISTERED NURSE CARE MANAGER (Registered Nurse 2) to join our Women's Health Care Center (WHCC) and our General Internal Medicine Clinic (GIMC), both located at our Roosevelt Clinic in the Seattle U-District.
SCHEDULE: The Registered Nurse Care Manager will work 40 hours per week, Monday - Friday.
RESPONSIBILITIES: The Registered Nurse Care Manager (RNCM) will play an active role in our General Internal Medicine and Women’s Health Care Centers at UWMC Roosevelt. The RNCM will be responsible for providing care management services for a panel of complex patients that receive their primary care at our clinics, including high-risk diabetes patients, pregnant women, adolescent patients, and adult patients with acute or chronic illness. The RNCM will support patients in avoiding hospital admission/readmission, accessing health care services, understanding their disease process and treatment plans, and setting realistic goals to achieve optimal health. The RNCM will use the nursing process to identify and prioritize healthcare triage interventions and lifestyle modifications. The RNCM is a key member of an interdisciplinary care team that coordinates and plans patient care.
A minimum of 2 years of Registered Nurse experience
An active Registered Nurse license in the State of Washington
Recent care management experience in an outpatient setting and/or nursing experience in primary care, internal medicine, or a similar practice area
Ambulatory Nurse or Care Management Certification
Recent telephone triage experience
