The Cardiovascular Center social worker plays a vital role in providing specialized psychosocial support for patients receiving cardiac care. This clinical social worker will partner with various teams to provide inclusive, collaborative care for patients with complex medical and/or mental health needs. You will be coordinating care with multidisciplinary teams to optimize patient care and outcomes. This position will offer psychotherapeutic interventions as well as case management services. Social work is critical to multiple patient care initiatives and programs including but not limited to: peer mentoring groups, support groups, and patient education.

You will be responsible for:
Completion of accurate assessments in a variety of situations
Collaborative relationships with multidisciplinary team members for patient care coordination
Provision of referrals and follow-up to community resources as needed
Excellent verbal and written communication skills and precise, timely documentation
Independent decision-making skills and prioritization of multiple, competing tasks
Participation in team and department meetings
Working individually or reciprocally to meet tangible and concrete needs.
Patient advocacy as needed
Provide optimal clinical practice skills, including some provision of short-term psychotherapy using evidence-based practice models, for adult patients
Partner on the peer mentoring programs and patient education initiatives
Offer support groups as needed
Master’s degree (MSW) in social work from a school accredited by the Council of Social Work Education
Current clinic license (LMSW) to practice social work in the State of Michigan, with clinical certification
LLMSWs will not be considered for this position
Capable of making effective, independent judgment calls in a variety of demanding situations
Must be flexible, adaptive, and able to prioritize competing demands in a fast-paced environment
Significant experience with medical social work, Motivational Interviewing, Trauma informed care, and experience delivering brief therapeutic interventions drawing on evidence-based psychotherapy models
This is a 32 hour/week position. Due to patient care needs, availability five days a week is required. There is flexibility on which two days are half day.
This position may be underfilled at a lower classification depending on the qualifications of the selected candidate. Advancement through the career ladder at the Expert or Clinical Scholar Level is required for placement in the MSW Senior Classification.
