This Per Diem Staff Nurse will be responsible for:
Answering and triaging calls from patients and families.
Assessing, planning, implementing, evaluating and modifying plans of care for a specific patient assignment throughout the healthcare continuum.
Contacting patients for changes in clinical management.
Providing ongoing care coordination between clinic visits by communicating with families, healthcare team members and other agencies involved.
Educating patients and families regarding disease management and medications.
Assisting with completion of plans of prescriptions, school forms and other documents as needed.
Providing oversight, delegation and act as a resource to unlicensed staff, including clinical assistants and patient experience representatives.
Working a minimum of 4, 8 or 10 hour shifts every 6 week schedule, Mondays-Fridays only- no holidays, with opportunity for more hours pending leaves and vacations
To qualify, you must have:
1 year of Pediatric RN experience in either inpatient neuroscience or in an ambulatory setting.
Ability to set priorities when organizing care for patients with varying acuity.
Strong telephone interview skills.
Collaborative teamwork abilities.
Knowledge of pediatrics and family centered care.
Ability to function autonomously using the nursing process.
Strong problem-solving skills, excellent oral and written communication skills.
Attention to detail, and a high degree of professionalism.
Ability to think creatively and work collaboratively to address problems.
