The Center for Integrative Health (CIH) supports health and wellness programs for Duquesne University’s campus and neighboring communities, and facilitates interdisciplinary education and research opportunities for students and faculty. The Bridges to Health Program (b2h) is a key program within the CIH that aims to address community-identified health priorities through sustainable, multi-sector partnerships. Under the direction of the Manger of Community Health Initiatives, the Community Health Worker Program Coordinator is responsible for managing the day-to-day operations of the b2h Community Health Worker (CHW) Program. The Community Health Worker Program Coordinator is responsible for overseeing CHWs to assist in the delivery of community-based services that address the social determinants of health, and bridge gaps between communities, individuals and health care providers.
This position involves coordination between CIH staff and community-based partner organizations. The coordinator must have strong interpersonal skills and be able to communicate effectively with a diverse population of patients, providers and community organizations. Work schedule may vary.
DUTIES AND RESPONSIBILITIES:
Oversees coordination and implementation of all CHW training programs. Support CHWs in the state certification process.
Oversees scheduling, management, marketing, and evaluation of all CHWs programs.
Supports CHWs in educating, encouraging, and serving as a role model for individuals and communities to adopt healthy behaviors, obtain recommended care, and provide up to date information on available resources.
Coordinates with social service agencies and healthcare providers to assist CHWs in meeting the needs of residents. The CHW will serve as an extension of the health care team and will be responsible for helping patients and their families navigate services and access community resources to improve health outcomes.
Assists CHWs in supporting patient access to community and governmental service agencies including help with completing applications and registration forms, as well as overcoming barriers to obtaining recommended healthcare, housing, food insecurity, unemployment, etc.
Supports CHWs in the development of time management skills, documentation, data collection, and goal setting.
Presents solution-driven approaches to supervisor and b2h team regarding barriers and unmet needs identified through the work of the CHWs.
Makes home visits, as appropriate, to assist CHWs and clients.
Prepares written reports and maintain records of service contacts and activities.
Participates in required training and staff activities.
Serves as a preceptor for and provide clinical education for Duquesne University Health.
Science students on clinical experiences in the Center for Integrative Health.
Performs other job related duties as assigned or required by the Manager of Community Health Initiatives.
Bachelor’s degree in social work or related human services field, master’s degree preferred.
Experience in health education and counseling preferred, or any equivalent combination of experience and training, which provides the knowledge, skills and abilities required to perform the essential job functions. This includes, but is not limited to, the following:
