The Nurse Coordinator will demonstrate a sound knowledge base and decision making ability in direct provision of effective and comprehensive nursing care for designated patient population.
Utilize the nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of the patient's care, as evidenced by patient/staff observation.
Assess the patient; collect and document the patient's current health status and medical history.
Plan/Implement; set priorities for nursing action in a logical sequence according to patient's needs and formulate a plan of care by the appropriate selection of Nurse Practice Standards. Plan includes physiological, psychosocial, and environmental factors: patient/significant other education and patient discharge planning.
Evaluate; perform ongoing assessment and revise plan of care based on new data and patient's condition.
Document accurate and ongoing assessment of patient status. Document patient care which reflects nursing intervention, patient response to care provided, patient needs, problems, capabilities, limitations, and progress toward goals. Documentation includes evidence of appropriate patient/significant other teaching, and the understanding of these instructions is noted in the medical record.
Communicate with the physician regarding patient needs, the nursing assessments, and recommendations on a daily basis, and as needed, as evidenced by patient feedback and administrative observation.
Collaborate with other nurses and health team members to ensure the patient's well-being as evidenced by staff/patient feedback.
Teach patient/significant other appropriate health information in a timely manner and share written informational material as evidenced by patient feedback and administrative observation.
Perform direct patient care, utilizing established procedures, policies, and standards as evidenced by patient/staff feedback and administrative observation.
Administer medications according to policy and procedure.
Administer IV's according to policy and procedure.
Observe and document patient's response to pertinent medications.
Demonstrate ability and appropriate technical skills when administering treatments and procedures in accordance with physician's orders and policy and procedure (done within a timely manner).
Demonstrate necessary skills and knowledge to provide care for patients according to policy and procedure.
Demonstrate ability to handle emergency situations in a prompt, precise, and professional manner.
Provide personal patient care to provide comfort and well-being to the patient, acknowledging physiological and psychological needs.
Perform indirect patient care, utilizing established procedures, policies, and standards as evidenced by patient/staff feedback and administrative observation.
Give a thorough, concise report at change of shift.
Communicate pertinent information to the appropriate person in a timely manner.
Demonstrate accurate and timely documentation according to form guidelines.
Accurately transcribe physician orders according to policy and procedure.
Maintain a clean, neat, and safe environment for patient and staff according to policy and procedure as evidenced by staff/patient feedback and administrative observation.
Ensure that supplies and equipment necessary for patient care are stored in an organized and efficient manner.
Demonstrate correct and safe technique in the use of equipment according to specific product information.
Follow appropriate procedure for obtaining and returning or cleaning/disposing of equipment and supplies.
Participate in economical utilization of supplies and ensure that equipment is maintained in a clean and safe manner.
Adhere to Administrative System/Nursing Service policies.
Remain conscientious in regard to personal hygiene and comply with specific division/unit regulations.
Comply with specific System/division/unit policy and procedure concerning safety.
100% attendance to mandatory skill updates yearly required.
Attend a minimum of four staff meetings and read and sign all minutes of meetings not attended.
Assist patients, family members or other patients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated.
Identify yourself in a pleasant and positive manner.
Take responsibility for helping the caller.
Assist in the orientation/training of new Team Members.
Consistently work in a positive and cooperative manner with fellow Team Members.
Assist other Team Members in the performance of their assignments.

Record/report the need for service maintenance or repair of equipment and remove any faulty equipment from service.
Contribute to the achievement of Performance Improvement and participate in PI activities.
Consistently evaluate work and determine if further steps are needed to meet patient expectations.
Take initiative to do or redo inadequate or incomplete work, even if it is not yours.
Ensure compliance with regulatory standards.
Adherence to System and Departmental Policies
Demonstrate knowledge and understanding of all policies and procedures and ability to reference them from appropriate books and manuals.
Comply with the System's infection control policies and procedures and the Exposure Control Plan which reflects OSHA Blood borne Pathogens Standard to ensure a safe working environment for self and others.
Ensure that the System is operated in an efficient and cost effective manner by adhering to System and departmental attendance policies as documented on the Employee Attendance Record.
Demonstrate adherence and support of System-Wide Service Standards as evidence by observation and patient feedback.
Demonstrate adherence to System/Department Complaint Management Standard.
Demonstrate adherence to System/Department Dress Code Standard.
Demonstrate adherence to System/Department Safe Practices Standards.
Requires a bachelor's degree or equivalent in education and experience, plus four years of related experience.
Active/valid New York State RN license.
BLS, ACLS, and PALS preferred.
Radiology (MRI, CT, and PET/CT) experience preferred.
Previous ER and ICU experience preferred.
