HARBORVIEW MEDICAL CENTER PHARMACY has an outstanding opportunity for an OUTPATIENT PHARMACIST – REFILL AUTHORIZATION CENTER (Full-Time, Rotating/Variable Shifts). The Outpatient Pharmacist - Refill Authorization Center (Pharmacist 2) performs medication prescription refill authorization under protocol, for patients of selected UW Medicine Clinics. The clinics, providers and patients the refill authorization center provides service for are from University Washington Medical Center Montlake and Northwest Campus, Harborview Medical Center, and University of Washington Neighborhood Clinics.
Clinical: • Review patient history; assess current drug therapy for appropriateness, compliance duration, and duplication of therapy. • Promote safe and effective use of medications and responsible medication management by screening patient profiles for medication efficacy and toxicity, drug interactions, appropriate laboratory monitoring, and patient adherence. • Ensure patient access to chronic medication refills in a timely manner. • Provide documentation in medical record and communication to prescriber as outlined in prescription refill protocol. • Maintain accurate medication histories and monitor for effect and adverse reactions. • Advise providers about patients’ drug therapy and recommend appropriate drug therapy based on patient parameters and interpretation of current literature, safety, efficacy, and cost. • Perform timely therapeutic interchange for specific classes of medications to meet insurance formulary demands.
Education: • Participate in regularly scheduled pharmacy continuing education provided by the department and or required for license certification. • Provide drug information when requested and appropriate to physicians, nurses, patients and pharmacy staff.
Other Duties: • Assist with periodic peer review and evaluation. • Have a Collaborative Drug Therapy Agreement for the UW Medicine RAC Services on file with the Washington State Pharmacy Quality Assurance Commission (PQAC)
