University of Iowa Healthcare is seeking a Board Certified physician in a Primary Care Specialty to serve as the Medical Director of Populations Health/ ACO and act as the Physician Champion in designing and implementation sustainable clinical processes structured to drive improvements that will have the highest impact on value base measure performance and savings. The Medical Director of Population Health / ACO is responsible for leading the clinical implementation of the UIHC Population Health Clinical Strategy which includes work in the following strategic quadrants: Preventive Care, Management of High-Risk Patients, Transitions of Care and Post-Acute Utilization Management.
This position will also have clinical responsibilities in a primary care department.
