We are seeking a compassionate, dedicated Nurse Practitioner to execute the following job duties with our inpatient Hepato-Pancreatico-Biliary and Liver Transplant population.
The Nurse Practitioner is responsible and accountable for the application of the nursing process, supervision of clinical practice, research involvement, consultative services, inter and intradepartmental liaison, education, leadership and patient care management across the health care continuum from preventative and acute care through discharge.
As an Nurse Practitioner, you will be responsible for evaluating the patients (includes: chart review, performing history and physical examinations, ordering diagnostic tests, reviewing results and reporting to the Physician as needed.
You will provide Inpatient services to Hepato-pancreatico-biliary (HPB) and Liver Transplant patients, coordinate daily activities of inpatient service, including facilitation of early discharge and care coordination/patient education, timely documentation of patient progress notes, discharge summaries, and CMS required care coordination notes.
You will coordinate communication of patient ADT and surgeries with referring and collaborating MDs.
You will develop and implement patient management plans and assist in provision of continuity of care.
The Nurse Practitioner serves as a patient advocate throughout the care process and represents the medical center at regional and national meetings/conferences.
There is a possibility of rotation in inpatient and outpatient services.
Master's of Nursing Degree (MSN) required
Graduate of an accredited Nurse Practitioner program required
Current State of California Registered Nurse and Nurse Practitioner License required
National certification as an NP required
Current ACLS certification from the American Heart Association required
DEA license required (or eligibility to obtain)
A minimum of 2 years of acute care nursing experience required
Experience as a Nurse Practitioner preferred
Experience with Transplant patients desired
