The APP will work with a multi-disciplinary team, including a patient care coordinator, to oversee all efforts related to guiding patients and families through the screening and enrollment process at UChicago Medicineâ™s Hyde Park and Orland Park sites. This may include some clinics during the evening and weekends.
Plans and develops patient centered programs, reinforces patient and provider education.
Develops and drives initiatives to increase screening in appropriate populations.
Communicates screening results with patients and referring physicians and coordinates referral services.
Acts as a liaison between Pulmonary, Thoracic Surgery, Radiation Oncology, Medical Oncology, and other physician practices and hospital departments the needs of screened patients.
Performs follow up management for screening patients.
Implements program guidelines, developing materials, workflows, and tools for effective communication.
Works closely with Cancer Service Line leadership and marketing teams to promote program outreach.
Evaluates goals and outcomes taking into account the benefit-versus-burden, safety, quality, and cost-effectiveness for the patient, family and the organization.
Develops diagnostic strategies and therapeutic interventions needed to achieve the goals and outcomes of the patientâ™s plan of care.
Performs other related work as needed.
An advanced degree (or equivalent, if license received prior to July 1, 2001) from an accredited school/college of nursing or completion of an accredited Physician Assistant educational program accredited by the Accreditation Review Commission on Education for the Physician Assistant, Inc. (ARC-PA).
Two or more years of clinical experience in a related area of specialty.
Licenses and Certifications for Advanced Practice Nurse:
Current Advanced Practice Nurse (APN) license in the State of Illinois or eligibility to receive by start date.Â
Current Registered Nurse (RN) license in the State of Illinois or eligibility to receive by start date. Â
Licenses and Certifications for Physician Assistant:
Current Physician Assistant license/registration in the State of Illinois or eligibility to receive by start date.
Certification from the National Commission for the Certification of Physician Assistants (NCCPA) by start date.
Demonstrated clinical competency, effective leadership and teaching skills.Â
Ability to conduct physical examinations and health assessments.
Ability to interview and obtain medical information.
Willingness to enter into written collaborative agreement with a collaborating physician upon hire.Â
Once licensed and as needed, willingness to apply for a controlled substance license and DEA number.
Willingness to be privileged and credentialed through UCM Medical Staff Office prior to start date.Â
Strong interpersonal skills and the ability to work independently.Â
