This Nurse Practitioner will be responsible for:
Working collaboratively with several cardiologists in coordinating care for patients requiring diagnostic evaluation and therapeutic interventions, including catheterization and surgical procedures, referred via the New England Regional network.
Providing care coordination and consultation across all Heart Center and hospital departments and collaborates with referring cardiology groups and local health care providers.
Providing continuity for patients and families during their care at Boston Children's Hospital via pre-procedure teaching and counselling, clinical management, education and psychosocial support during their hospital stay, and post procedure follow up as needed.
Assuming responsibility for the flow of health information between the referring cardiology groups and Boston Children's Hospital.
Participating in all aspects of the management of patients in the collaborating physician's caseload including admitting and discharging patients, placing orders and writing prescriptions, as well as the assessment and management of common health problems in infants and children with primarily congenital heart disease in the inpatient and outpatient settings.
Approximately 50% direct care and 50% care coordination time, including care coordination in Cardiac Pre-op.
Working 40 hours per week, 5 days, Monday-Friday. Will consider four 10-hour days, Tuesday-Friday.
To qualify, you must have:
MSN and Massachusetts license as Pediatric or Family Nurse Practitioner.
3-5 years' Nurse Practitioner experience in acute care pediatric cardiac care. Will consider new grad NP with 3-5 years' RN experience in acute care pediatric cardiac care.
Exemplary clinical skills to provide direction to the care team.
Ability to prioritize and function in dynamic environment
Analytic skills to make effective patient care decisions
