The Social Worker is responsible to the Nurse Family Partnership Nursing Director. the Social Worker (LCSW), interacts with the client as part of the primary care team. The LCSW:
Provides services to clients and families.
Engages in crisis intervention with the primary care team in the home.
Provides ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual client needs both in office and in the client’s home.
Enhances the quality of the client management and satisfaction through the integrating and functions of case management, utilization review, and goal planning.
The LCSW is responsible for having accountability for the care, coordination and goal planning of all clients; having accountability to the Nursing Director; and other related duties as assigned. The LCSW will work to address other social needs of the client and will utilize other area referral sources for needed specialty care.

Must be a Licensed Master of Social Worker who possesses current Tennessee licensure as a Licensed Master Social Worker
Must be eligible for credentialing with insurance companies throughout employment.
Must adhere to HTPAA regulations; must maintain active licensure; must adhere to NASW Code of Ethics; must maintain professional competency by participating in continuing education programs, reviewing current literature and research.
Must have current CPR certification upon hire.
