Position plans, organizes and implements social work services for patients and families.
PRIMARY DUTIES AND RESPONSIBILITIES:
Assists with decreasing readmission rates.��
Assists with finding low-cost medication resources.����
Provides telephone follow-up for post-discharge patients that miss their appointments, and reschedules as needed.�������������������������������������������������������
Facilitates referrals and community resources for patients/families.��������������������
Assists patient with enrolling in Medicaid or other health insurance programs.����
Helps patients navigate the financial help aid at Wash U and BJC.�
Collaborates effectively with healthcare team.
Assists with transportation issues.�
Verifies insurance coverage for patients.
Answers inquiries from patients/families about community resources.����
Master’s degree in Social Work required.
REQUIRED LICENSURE/CERTIFICATION/REGISTRATION: Currently a Licensed Master Social Work (LMSW) or Licensed Clinical Social Worker (LCSW) in the State of Missouri, or Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in the State of Illinois. Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements).
Preference given to community experienced social workers.
Working knowledge of computers.
Good communication skills (both verbal and written).
Strong organizational and interpersonal skills.
The hiring range for this position is $51,272 - $65,374 annually.
