The Nurse Practitioner, in concert with the physicians, is responsible for constructing thoughtful differential diagnoses and tailoring individualized and evidenced-based treatment plans. S/he integrates the principles of preventive medicine and patient education into all aspects of clinical care.
Assists physician with assessment, evaluation and management of patients.
Communicates all clinical activity to supervising physician.
Performs H&P; documents of such per institution standards
Assesses patients using all available physical and diagnostic data, incorporating medical history and evaluate patient status and progress with current plan of care
Write orders for patients as allowed by state law e.g. prescribe medications, excluding chemotherapy, non-pharmacologic therapy, diagnostic tests, cultures, or infusion procedures
Initiates approved protocol/plan
Manages patients with chronic illness requiring repetitive treatments, (chemotherapy/infusions) and prepares orders and manages all aspects of patient care, including outpatient follow-up, from initiation of therapy
Monitors and assesses patients during and after selected therapies.
Attends staff meetings, patient support meetings, and continuing education pertaining to specialty as appropriate
Participates in all quality improvement/process improvement initiatives
Obtains insurance authorizations and performs peer reviews as is appropriate
Performs patient and family education and counselling including disease state, management/plan of care, side effect, medications, all follow up
Performs phone triage and provides phone follow up/direction to patients as is appropriate to the situation
Performs other duties as assigned.
Bachelor's Degree, Or equivalent plus four years in Nursing is required and must hold a NYS License as a Nurse Practitioner.
Working knowledge of provider billing and documentation requirements is a must.
Ability to work with both Pediatric and Adult patients.
Demonstrated ability to work well with physicians.
Ability to work under pressure with multiple and competing deadlines required and have excellent communication skills, written and oral.
Must be highly organized, with excellent communication and follow through skills.
Must have proven record of ability to work as part of a team, with administrative, clinical research, and financial staff.
Experience utilizing an EMR.
Urology, surgery and or pediatric specific experience highly preferred.
