What will I be doing in this role?
A Psychiatric Social Worker (PSW) collaborates with an interdisciplinary team in providing care to the psychiatric patient and is a resource for all caregivers, the patient and the patient's family. In conjunction with the licensed independent provider, provides education and information on available options, the informed decision making process, and realistic goal setting in order to empower the patient/caregiver to actively participate in their plan of care. Establishes and coordinates a supportive care plan with goals and interventions based upon treatment and individual patient/caregiver needs, preferences, beliefs and values. Keeps patient/caregiver involved in plan and moving toward self-directed care. Reviews, evaluates and revises the plan of care for appropriateness on an ongoing and timely basis. The PSW is an advocate for the patient/caregiver. The PSW assesses the ability of the patients to follow instructions and give consent. He/she identifies safety concerns and institutes safety measures. Facilitates multidisciplinary conferences when indicated by complexity of patient's psychiatric condition or other barriers to care:
Assists in the assessment of "holdable" status and in decision making related to restraint use. Writes and removes 5150 hold.
Coordinate all transfers to psychiatric facilities, both voluntary and involuntary, including attainment of insurance authorization, where applicable.
Complete Columbia Suicide Severity Rating Scale (CSSRS) as indicated and provides appropriate referrals
Develop collaborative plans for high-utilizers with psychiatric comorbidities.
Proficiency navigates community systems of care. Provides appropriate resources and referrals based on individual need.
Provides Continuing Your Care (CYC) referrals to Saban Clinic.
Observes, evaluates and expeditiously alters plan of care based on signs and symptoms of psychiatric distress.
Identifies psychiatric level of care for appropriate, safe, and complete transition of care. Plans and implements individualized patient care in collaboration with interdisciplinary teams based on observations. Implements appropriate reporting, referrals, and care in accordance with standardized procedures. Initiates emergency procedures when indicated. Assists in discharge planning and act as a liaison between ED team, PET team, and Transfer Center.
Provides care to special patient populations and patients with diverse cultural backgrounds. Provides crisis support and therapeutic counseling
Identifies and make special adjustments as required to the specific populations needs, including cultural, spiritual, age, psychosocial, communication, gender, sexual orientation, economic, education, family and condition needs. Provides education for patient to navigate community support.
Identifies and assesses patient safety concerns with respect to age and developmental considerations. Intervenes to provide a safe environment and evaluates effectiveness of intervention for patient.
Provides assistance to co-workers, new staff, temporary staff and students. A resource to staff for psychiatric related clinical questions/concerns.
Works collaboratively with all ED staff, Social Workers, Case Managers, Community Resource Coordinators (CRC's). Consults with Psychiatry
Consults with DMH for referrals to outpatient clinics.
