UW Medical Center – Northwest has an outstanding opportunity for a full-time (100% FTE, 40 hours per week), day shift REGISTERED NURSE to join our Primary Care Team in Lake Forest Park. The Registered Nurse will play a critical role providing ongoing case management and coordination through UW Medicine’s continuum of care.
SCHEDULE: The Registered Nurse will work 40 hours per week, day shift only.
RESPONSIBILITIES: As a Registered Nurse, you will follow evidence-based nursing practices, exercise independent judgement, and manage complex primary care health issues. You will collaborate with patients, their family, and other healthcare providers to develop and implement treatment plans. You will interact regularly with providers and support staff across the UW Medicine system, including pharmacies, hospital personnel, laboratories, and home care nurses. Additionally, you will provide assessments, planning, education and evaluation of the patient’s response to treatment, and will deliver services through scheduled in-person visits and via telehealth.
3 years of experience in care management, home health care, or primary care/triage
Current license to practice as a registered nurse in the State of Washington
Bachelor’s degree in nursing (BSN)
Ambulatory Nurse or Care Management Certification
