Job Description Summary
This full-time position is for a Senior Certified Recovery Specialist (CRS) who will be a core member of a collaborative care team in primary care. The team includes the patient's primary care provider, a social worker who provides mental health care, and a psychiatrist who consults to the team remotely. The Senior CRS will be part of a research study evaluating collaborative care models for integrating buprenorphine treatment and mental health care in primary care for patients with opioid use disorders and mental health conditions. This individual will supervise the work of other CRSs and serve as a liaison between the other CRSs and the primary care team and research study. The Senior CRS will assist in training of the CRSs and with project implementation. This person will develop relationships with community groups/agencies in partnership with others in the agency. The Senior CRS will be responsible for updating and expanding resources within Penn and with community practices (communities of recovery, educational, vocational, social, cultural, spiritual resources, mutual self- help groups, professional services, etc.). This will involve a collaborative effort including the recovering person, agency staff and other relevant stakeholders. This individual will also assist with study recruitment by maintaining communication with clinical partners to raise awareness of the study and enhance referrals. The Senior CRS will also be responsible for managing their own caseload of patients by providing patients with recovery education and by providing a model for both people in recovery and staff by demonstrating that recovery is possible. The Senior CRS will maintain contact either in-person, by phone or e-mail with the recovering person once they start the study and throughout the intervention to ensure ongoing success and to provide re-engagement support in partnership with others in the agency if needed. This individual will assist the recovering person to identify their personal interests, goals, strengths and weaknesses regarding recovery and link them to needed resources. The Senior CRS will help the other CRSs to identify barriers to full participation in recovery-related activities and develop strategies to overcome those barriers for their caseloads. The target population are individuals with opioid use disorders and mental health conditions.
This position is for a Senior Certified Recovery Specialist (CRS) who will take a lead role in providing support for a complex study that evaluates the effectiveness of collaborative care models in primary care for patients with mental health and opioids use disorders. The primary duties for the Senior CRS will include, but are not limited to:
Meet with their own caseload of patients regularly to assess treatment adherence, address patient needs and learn about unmet needs. Accompany patient to primary care and other health care visits as needed and provide other services such as picking up and delivering medications.
Follow patients throughout the care continuum, facilitate and expedite appointments for follow-up and acute care, consults and support services including behavioral health clinicians, social workers, physicians, community agencies, etc. Serve as an essential link between patients and all other care providers and clinical and research staff.
Serve as a liaison with the research and clinical care team. Coordinate multidisciplinary team meetings; provide updates on activities of CRSs and develop concise patient progress summaries for use by the care and research team, and document recommendations made utilizing standardized care protocols in accordance with state/nationally recognized care guidelines.
Provide oversight and supervision to other CRCs to ensure that the CRSs are providing a collaborative and coordinated approach that incorporates recovery principles. Assist CRSs as they provide warm handoffs. Support the CRS as they work to assess, identify, engage and help individuals transition and follow through with treatment and support services.
Assist in the training of the CRS and with the implementation of the protocol at project sites.
Maintain and expand communication with clinical partners to raise awareness of the study and enhance referrals.
Outreach to Penn departments and community partners to update and expand resources. Establish and maintain relationships with Philadelphiaâ™s behavioral health systems and targeted organizations and utilize those relationships to maintain a working knowledge of surrounding resources including visiting referral facilities.
Monitor and evaluate the delivery of care through documentation and tracking of needs addressed, track outcomes and support strategic planning through data collection in accordance with all state requirements and the research protocol

High School Diploma and 3-5 years of experience or an equivalent combination of education and experience required. An Associateâ™s Degree in behavioral health, nursing, or related field is preferred. Must have 2 or more years of prior behavioral health experience and knowledge of behavioral health resources. Must be a Certified Recovery Specialist (CRS). Must maintain a valid CRS certification and remain in good standingÂ during employment.
