Under the direct supervision of the Pharmacist, the Medication History Technician coordinates the medication history assessment and documentation into the medication record. This position The medication history technician will work together with inpatient pharmacists, nurses, and other healthcare personnel in order to complete the necessary functions to improve transitions of care for inpatient admissions and discharge. In addition to these specialized responsibilities, the medication history technician may also assist pharmacists in preparing and distributing drugs, maintaining the drug inventory and maintenance of records.
Minimum Education:â¢High school diploma or equivalent education.â¢Completion of Accredited Pharmacy Technician Certificate ProgramMinimum Experience/Knowledge:â¢Minimum of three years of Pharmacy Technician experience required. â¢Previous retail (outpatient) pharmacy experience including prescription transcription duties highly preferred. â¢Trained in general use of pharmacy computer systemsâ¢Requires a working knowledge of community pharmacy practice including some medication therapy management, basic to advanced medication knowledge and medication history taking.â¢Requires an ability to interpret medical and pharmacy terminology, medication names, prescribed doses and frequencies, available dosage forms, prescription insurance billing and formulary substitutions, communicating with pharmacy resources to confirm dispense history and problem solving skills.â¢Experience in preparing medications for Pharmacist review and placing automated medication orders.â¢Experience in reporting and recording adverse medication reactions.Required License/Certification:â¢Current registration as a Pharmacy Technician with the California State board of Pharmacy required.â¢Pharmacy Technician Certification Board (PTCB) certification preferred.
