JOBSUMMARY/ESSENTIAL JOB FUNCTIONS: Under the supervision of the AssistantVice Chancellor, the Case Manager works with the Behavioral Intervention Team andserves as an institutional point-of-contact for the mental health needs ofstudents. This position assists in coordinating prevention, intervention, andsupport efforts across campus/community systems to assist at-risk students andstudents facing crises, life traumas, and other barriers that impede success.
Serves as a member of the Behavioral Intervention Team (CARE Team), provides overview of student cases, and serves as a recorder.
Serves as a point-of-contact, as necessary, in triagingstudents to support services, including counseling; ensuring continuity of careand coordinates and tracks referrals to internal units such as Student AcademicSupport Services and Inclusion, University Health Services, Student Conduct,Behavioral Intervention Team (CARE Team), and other University offices, asdirected.
Compiles statistical data and prepares reports for program operation, activities, and progress, as requested.
Designs and implements individual action plans for identified students using evidence-based practices.
Coordinates culturally responsive services to underserved populations (e.g. suicide, sexual assault, alcohol and drug treatment services, etc.).
Collaborates with the Assistant Vice Chancellor and/or Behavioral Intervention Team (CARE Team) to track and record measures of student success and works with the Assistant Vice Chancellor to coordinate prevention and outreach, as necessary.
Establishes and implements methods to evaluate the effectiveness/success of the program.
Facilitates broader resource linkages for students such as medication management and longer-term counseling services.
Inputs, maintains, and updates clinical documentation/information regarding cases and inquiries into respective databases.
Compiles and maintains a directory of community resources and develops on-going referral relationships with community partners.
Supports students re-entering the University following Leave of Absences or Withdrawals.
Maintains accurate electronic case management records.
Performs other related duties as assigned.
