Reporting to Keck Medicine of USC, Department of College Health, incumbent assists with all aspects of patient care for USC Student Health Services serving as a Care Coordinator and primary contact in coordinating services at Keck Medicine of USC and Student Health services for Engemann Student Health Center and Cohen Student Health Center.Â This includes being the liaison between Keck and Student Health workflow processes; accessing the applicable Keck Medicine of USC outpatient practices to assure timely access and coordination of patient records.Â Â Individual takes accountability for the patientsâ beginning-to-end experience regardless of their needs. Â Assists Student Health care givers and patients in eliminating barriers to care by referring to community resources and/or appropriate staff members. Â Assists/coordinates patient appointments and follow up. Â
Assists multidisciplinary team in managing the care of the patient thru all phases (In-take/Evaluation/Listed) by acting as a liaison between the clinic and referring physicians; assists in managing follow-up care office visits including post-surgical patients by setting up follow up care appointments; and/or coordinating an organized and efficient case flow of clinic scheduling.Â Assists the office with clerical duties and other duties as assigned.
Must have excellent communication skills, including the ability to speak, read and write English proficiently. Must be comfortable with computers and medical terminology. In depth knowledge of all insurance types.
Minimum Education:High school graduate or equivalent Bachelorâs Degree preferred.Minimum Experience/Knowledge:2+ years experience in acute care or ambulatory care setting preferred. Knowledge of medical terminology preferred. Familiarity of word processing, Microsoft Outlook, , Cerner, PBAR and scheduling software is preferred.Must have excellent communication skills, including the ability to speak, read and write English proficiently.
