The Navigation Nurse Manager manages and oversees a program of healthcare navigation designed to assist the patient and family across the care continuum.Â The navigation team is comprised of licensed and non-licensed personnel who serve to guide patients through the healthcare system and help to overcome barriers to care.Â Under the direction of USC Care and Ambulatory Services, and in compliance with organizational goals and appropriate regulatory agency requirements, the navigation team collaborates with a multi-disciplinary team to allow for timely evaluation, treatment, and supportive care across all touch points in care. The Manager is accountable for quality and continuity of patient care, efficiency of operations, and human resources management for the assigned clinics or service lines.Â Directs nursing practice, education, and services.Â Serves as a personal contact for physicians, staff, and outside departments in order to address problems, issues or concerns that require attention.Â
The Navigation Nurse Manager promotes a positive and professional environment; supervises clinical staff, maintains a stable and productive staff, involves staff in decisions that impact the clinics; makes data driven decisions; other duties as assigned.Â .Â
Minimum Education:â¢Bachelorâs Degree in Nursing required.â¢Masterâs Degree in Nursing or in progress (strongly preferred)â¢*Masterâs Degree in Nursing may be in lieu-of Bachlorâs Degree in Nursing.Minimum Experience/Knowledge:â¢Five (5) years experience as a RN required with at least two (2) years experience in a formal management position required.â¢Experience in the field of nurse navigation preferredâ¢Experience in Acute or Ambulatory setting preferred.â¢Strong verbal and written communication skills.â¢Strong commitment to patient confidentiality.â¢Demonstrated ability to listen and be a liaison between the patient/family/caregiver and the medical providers, in developing a plan that is medically sound and culturally acceptable.â¢Must be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies. Proficient skills in computer applications.Required License/Certification:â¢Current and active California RN License required. â¢Current BLS certification for Healthcare Provider required.â¢Fire and Safety Certification - If no card upon hire, one must be obtained within 30 days of hire, and maintained by renewal before expiration date.
