The Novel Interventions in Children's Healthcare (NICH) Program at OHSU is currently hiring for the position of Interventionist in Portland, Oregon. The position is housed within theWomen's and Children's Servicesat Doernbecher Children's Hospital in Portland, travel will be required. The position is a good fit for individuals who enjoy a blend of direct service and system-level intervention, and have experience and interest in working with at-risk youth or youth with chronic/complex medical conditions.
The NICH program provides services to children and adolescents with medical complexity (ex:type 1 diabetes, cystic fibrosis) and psychosocial vulnerability (ex: housing and food insecurity, child welfare involvement). NICH is designed to address the triple aim of healthcare reform (reduce costs, improve care, and improve health) by providing intensive inpatient and outpatient support to youth and their families with the aim of improving health management in the context of the youth's daily life. NICH services focus primarily on youth covered by Medicaid.
The purpose of this position is to serve as an Interventionist to families eligible for the NICH program. Interventionists provide services including 1) strategic family and systems-level interventions, 2) health systems integration and patient needs alignment, and 3) resource access for family and patient optimization. Duties also include conducting home visits, attending medical appointments, and providing 24/7 on call support.
A Bachelors degree with preferred major course work and experience in a relevant field (ex: psychology, nursing, social services, public health, counseling or allied health profession).
A strong backgroundin working with at-risk youth and families and/or patients with chronic health conditions. A minimum of two years is preferred.
Interventionists transport patients and families in their personal vehicles. Having a car with valid insurance is required for this position.
