DUTIES: The Department of Pharmacy at SUNY Downstate Health Sciences University is seeking a full-time Pharmacist. - Provide comprehensive clinical and operational pharmacy services, such as drug information, therapeutic drug monitoring, electronic order verification, and sterile and non-sterile compounding and compounding of hazardous drugs such as chemotherapeutic agents. - Evaluate and monitor pharmacotherapy and recommend adjustments to medication regimens by utilizing appropriate pharmacokinetic/pharmacodynamics principles, evidence-based medicine, and patient- specific factors. - Collaborate with medical, nursing, and pharmacy staff to triage and resolve medication-related and pharmacy automation- related issues. - Oversee activities of pharmacy technicians and hospitals attendants, and redirect workflows as needed to meet patient care needs. - Assist in the proper handing, dispensing, and storage of controlled substances. - Detect and report medication safety events and adverse drug reactions. - Serve as member of internal pharmacy- specific committees and participate in departmental performance improvement initiatives. - Precept and mentor PGY1 pharmacy residents during weekend and evening pharmacy practice experiences, as well as IPPE pharmacy students from local colleges of pharmacy. - Participate in additional clinical, operational, and/or education pharmacy activities based on the candidate's professional interest, as under the direction of the Director of Pharmacy or Associate Director of Pharmacy.
