This Staff Nurse/Care Coordinator will be responsible for:
Joining the Boston Adult Congenital Heart (BACH) Program in this exciting opportunity as Nurse Coordinator.
Collaborating closely with a multidisciplinary team including cardiologists, advanced practice clinicians, social work, administrative staff and other multidisciplinary healthcare team members, providing care for the adult patients with congenital and acquired heart disease.
Coordinating care with referring clinicians, patients, and families to ensure ease of access and ongoing communication, continually striving to enhance the BCH experience and assure safe, effective, efficient, family centered care.
Maintaining a relationship with the patient/family throughout the course of their care.
Updating and maintaining a strong relationship with referring physicians.
Partnering with administrative and clinical team to support and improve day to day operations and future expansion efforts.
Assisting with scheduling coordination and clinic session preparation.
Supporting longitudinal care needs including prescription refills, phone triage, assisting with prior authorizations, provision of anticipatory guidance and patient/family education.
Facilitating preadmission scheduling and care coordination and discharge preparation, including collaboration with community agencies.
Capturing, tracking, and developing reporting mechanisms on key quality indicators.
Act as a resource and mentor to other Cardiac Center staff regarding patient or family processes or procedure inquiries, and complex patient issues.
Working in Boston and satellite locations.
To qualify, you must have:
Bachelor of Science degree in Nursing and Massachusetts RN License.
BLS, ACLS certification.
At least three (3) years of prior adult cardiac nursing experience.
Experience in adult CICU or acute care inpatient setting preferred.
Proficiency in Microsoft Word and Excel; EMR competency.
Excellent Customer Service skills; strong written and oral communication skills.
Excellent organizational and documentation skills.
Experience with interdisciplinary health care approach.
Ability to complete multiple tasks and work under pressure.
