Rutgers, The State University of New Jersey is seeking a Community Coordinator for the Social Services Department at Eric B. Chandler Health Center of the Rutgers Robert Wood Johnson Medical School.
The Community Coordinator will be responsible for helping patients and their families to navigate and access community services, other resources and adopt healthy behaviors. The position supports medical and behavioral health care providers through an integrated approach to care management and community outreach. As a priority, activity will promote, maintain and improve the health of patients and their family.
Among the key duties of this position are the following:
Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement.
Through the use of motivational interviewing and other evidence based strategies, assists patients and families with goal setting.
Conducts intake interviews with patients, including enrolling and/or referring patients into relevant programs that enhance wellness.
Follows-up with patients via phone calls, home visits and visits to other community settings.
Conducts eligibility determinations, enrollments and follow-up with uninsured patients.
Works closely with medical and behavioral healthcare providers, nursing staff, registration staff and other personnel to help ensure that patients have comprehensive and coordinated care.
Minimum Education and Experience:
High School Diploma or equivalent.
Required Knowledge, Skills, and Abilities:
Fluent in Spanish and English.
Experience working in a multi-cultural setting.
Basic computer skills.
Ability to initiate and maintain positive working relationships with staff and other organizations.
Understand the community served – community connectedness.
Good communication skills, such as listening well, and using language appropriately.
Ability and willingness to provide emotional support, encouragement and motivation to patients.
Successful completion of a Community Health Worker formal training program from a college or other educational institution.
Experience working in a community-based setting for at least 1 to 2 years.
Knowledge of some medical terminology.
