For this project, we are seeking to hire a full-time Re-Entry Health Navigator who will be primarily based in one of the following counties: Rowan, Bath, Morgan, Menifee, Elliott, Lee, Owsley, Wolfe, Perry, Letcher, Leslie, Knott.
The project involves delivering an intervention to reduce substance use disorders and related harms among people re-entering the community from rural jails, as well as working with community coalitions who will guide the implementation of the intervention.
Job responsibilities include (but are not limited to) screening prospective participants for eligibility, implementing the intervention, scheduling and conducting surveys and interviews with individuals who use drugs and other community members, conducting rapid HIV and HCV testing (oral or finger stick) after becoming trained and state certified, and collaborating with coalitions who guide the implementation of the evidence-based intervention in their county.

BA plus 2 or more years of equivalent experience preferred. Driver's license preferred.
