Required Skill/ability 1: Strong communication skills to assess patient/family health information, preventive services and screening needs in-person or by telephone to determine psycho-social and logistic barriers to healthy behaviors including timely cancer screening. Fluency in English, both written and verbal.
Required Skill/ability 4: Ability to maintain organized records, document all client interactions, work independently and communicate effectively.
Required Skill/ability 2: Ability to schedule appointments and coordinate connections with prevention and health promotion services (e.g. tobacco treatment programs, nutrition counseling) and with programs/services (e.g. transportation, emergency coverage, food assistance) to enable timely follow up for patients who need cancer treatment or care.
Two years of related work experience and a high school level education; or the equivalent combination of experience and education.
