This Staff Nurse will be responsible for:

• Answering and triaging calls from patients and families.
• Assessing, planning, implementing, evaluating and modifying plans of care for a specific patient assignment throughout the healthcare continuum.
• Contacting patients for changes in clinical management.
• Providing ongoing care coordination between clinic visits by communicating with families, healthcare team members and other agencies involved.
• Educating patients and families regarding disease management and medications.
• Assisting with completion of plans of prescriptions, school forms and other documents as needed.
• Providing oversight, delegation and act as a resource to unlicensed staff, including clinical assistants and patient experience representatives.
• Working four 10-hour shifts, Monday - Friday.

• BSN degree.
• 1-2 years of Pediatric RN experience in either inpatient neuroscience or in an ambulatory setting.
• Ability to set priorities when organizing care for patients with varying acuity.
• Strong telephone interview skills.
• Collaborative teamwork abilities.
• Knowledge of pediatrics and family centered care.
• Ability to function autonomously using the nursing process.
• Strong problem-solving skills, excellent oral and written communication skills.
• Attention to detail, and a high degree of professionalism.
• Ability to think creatively and work collaboratively to address problems.
