The UNC Center for Excellence in Community mental health, part of the Department of Psychiatry at the University of North Carolina at Chapel Hill, seeks to hire a full-time, fixed-term Clinical Instructor. The position will be part of the case management team and Wellness on Wheels, working closely with Center's community programs based in Carrboro providing assertive case management services in the community. Case Management Services include support for coordination and support for services such as housing assistance, financial assistance, legal assistance, and financial counseling. Case Manager will offer clinical case management, motivational interviewing, shared decision making, and assist with real world problem solving.
Assure that clients are appropriately connected to the community-based services.
Develop cease management plans based on assessments
Assist with accessing appropriate and cost-effective services and/or supports in the community;
Assist with addressing barriers to healthcare including but not limited to financial, transportation, appropriate housing.
Access supported employment needs and make appropriate referral to IPOS supported employment providers
Assist with completing social determinants of health questionnaires
Assist with care management activities to make sure clients are receiving appropriate level of care.
Attend treatment team meetings
Supervising and training of students and interns rotating through the department. The office is located in Carrboro, NC.
Education and Experience:
Direct work experience with persons with severe and persistent mental illnesses. Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for client rights and personal preferences in treatment and services are essential. Knowledge of mental illness and mental health and social service systems, ability to navigate complex systems and to participate as member of multidisciplinary team required.
