Required Qualifications (as evidenced by an attached resume):
A Diploma from an accredited school of Nursing or a college degree in Nursing. New York State Registered Professional Nurse. Two (2) or more years of full time case management experience. Must have strong knowledge of MS Suite and with an Electronic Medical Records system.
Bachelor's Degree (foreign equivalent or higher ) in Nursing. Additional years (3+) of full time case management experience. Two (2) years full time nursing experience in either a hospital, ambulatory or medical office setting. Experience with assisting cancer patients. Experience with 9/11 Responders or the first responder population. Experience with developing and implementing treatment plans or Plan of Care notes. Mental health experience, particularly in post-traumatic stress or depression. Experience with chronic illnesses. Bilingual (Spanish/English). Experience with Workers' Compensation.
Brief Description of Duties :
The Medical Case Manager must possess the knowledge and experience to assess, plan and intervene as needed, to promote health, secure services and help patients cope with serious health issues. The incumbent must be able to work independently and as a member of a team. S/he must be detail oriented and have strong oral and written communication skills. S/he will be responsible for the following duties:
Responsible for care/case management, primarily for World Trade Center Health Program (WTCHP) patients with complex aero digestive health conditions, mental health conditions, and/or cancer.
Assist Member Services and Case Management Administrator with the continuous development of systems within the program. This includes special projects and workgroups that serve to assist our population.
Special projects include assisting in creating an Instensive Case Management support group and a Mental Health compliance program.
Assist with evaluation and measuring quality of case management team.
Educate patients re: WTC Health Program benefits and limitations.
Perform appropriate referral and disposition planning.
Pre-authorize covered tests, procedures and treatments.
Provide support services to patients and families.
Assist in the clinic as needed for assessments and medical /mental health screenings in order to refer our members for services and treatment.
Other duties as assigned.
