The University of Southern California (USC) School of Pharmacy invites applications for a staff ambulatory care clinical pharmacist based at Keck Hospital of USC. Priority will be given to candidates with expertise in the implementation, development, and refinement of ambulatory care programs, including quality improvement and outcomes tracking. Practice includes comprehensive medication management for common ambulatory care conditions (e.g., diabetes, hypertension, thromboembolic disorders, heart failure, care transition hand-offs at discharge) with opportunities to develop new programs that are high-value to Keck enterprise. The position includes participation in management and planning meetings related to the pharmacy ambulatory care program and precepting APPE and IPPE students at all levels (1-4).
Required Credentials and Experience:
Applicants must have a PharmD degree, and licensure or eligibility for licensure in California. Completion of a pharmacy residency and/or fellowship or formal care transitions program management experience is preferred. A record of professional leadership and evidence of engagement in scholarship is expected. Experience in clinical practice site development / advancement is preferred.
The USC School of Pharmacy is located on the USC Health Sciences Campus, which includes the Keck Hospital of USC, the Norris Cancer Hospital, and LAC+USC hospital. The Keck Hospital of USC is a 401-bed acute care hospital. Adjacent to the Keck Hospital are the Healthcare Centers 1, 2, and 4 which house a variety of outpatient primary care and specialty practices.
Minimum Education: Bachelor's degreeMinimum Experience: 1 yearMinimum Field of Expertise: Degree from accredited (ACPE) school of pharmacy. Registered pharmacist in state of California. Hospital pharmacy experience. Familiarity with unit dose, IV admixtures and clinical pharmacy practice.
