Rutgers, The State University of New Jersey is seeking an Advanced Practice Nurse for the Department of Family Medicine & Community Health at Rutgers Robert Wood Johnson Medical School.
The primary purpose of the Advanced Practice Nurse is to provide geriatric care to patients at various locations including outpatient, assisted living facility, skilled nursing home and home visits while functioning independently and working collaboratively with physicians of the Rutgers Robert Wood Johnson Medical School. The specific protocols of treatment and care plan are developed and carried out by the Advanced Practice Nurse and collaborating physician through a practice agreement. The Advanced Practice Nurse provides consultation to clinical staff regarding patient care issues and participates in a collegial relationship with the physicians, nurses and other health care professionals. This position requires a high level of professional responsibility, medical judgment, and ability to take personal initiative while working as part of a team. Among the key duties of this position are the following:
Performs physicals, assessments, takes medical histories, implements appropriate diagnostic and therapeutic plans in partnership with collaborating geriatrician.
Develops a treatment and care plan either independently or as part of an interdisciplinary team along with care planning.
Informs and instructs patients/significant others on patient’s health/illness state and plan of care.
Documents all aspects of patient care and maintains proper medical records in Electronic Medical Records (EMR) system.
Optimizes the function and quality of life of those with multiple chronic and incurable conditions through understanding of the interdisciplinary approach to care.
Effectively relates and presents cases at Geriatric Grand Rounds.
Minimum Education and Experience:
Certified as either a Clinical Nurse Specialist or Nurse Practitioner in the State of New Jersey.
At least two (2) years of related nursing experience, preferably as an Advanced Practice Nurse.
Required Knowledge, Skills, and Abilities:
Must maintain current CPR for healthcare provider certification.
Demonstrated experience in treating a diverse patient population.
Bilingual skills in English/Spanish.
