Rutgers, The State University of New Jersey is seeking an Advanced Practice Nurse for the Department of Family Medicine & Community Health at Rutgers Robert Wood Johnson Medical School.
Working collaboratively with a physician and under the clinical supervision of the Home Visit Service Chief, the Advanced Practice Nurse (APN) provides medical evaluations and ongoing medication treatment to patients. The specific protocols of treatment are worked out by the Advanced Practice Nurse and the collaborating physician through a collaboration agreement. The Advanced Practice Nurse provides consultation to clinical staff regarding patient care issues, and may also provide formal in-service education and training to other employees. The Advanced Practice Nurse may work in multiple programs and different sites on different days. This position requires a high level of professional responsibility, medical judgment, and ability to take personal initiative while working as part of a team. Among the key duties of this position are the following:
Participates in a collegial relationship with the physicians, nurses and other health care professionals.
Performs in an expanded nursing role and has responsibility for meeting the needs of a determined number of patients using a primary care model.
Participates in data collection related to the patients’ status and needs.
Informs and instructs patients/significant others on patient’s health/illness state and plan of care.
Documents all aspects of patient care in EMR system.
Administers medications and parenteral therapy in accordance with established policies.
Reports all necessary information and/or unusual occurrences in accordance with established policies and procedures.
Participates in staff development and educational activities.
Minimum Education and Experience:
Certified as either a Clinical Nurse Specialist or a Nurse Practitioner in the State of New Jersey.
At least two (2) years of related nursing experience.
Demonstrated experience in treating a diverse patient population.
Required Knowledge, Skills, and Abilities:
Must maintain current CPR for Healthcare Provider Certification.
Appointee will be required to possess a valid driver's license with a clean driving record as the operation of a vehicle is necessary to perform the essential duties of the position and to travel to different sites.
Related experience as an Advanced Practice Nurse.
Bilingual Skills in English/Spanish.
Use of personal automobile is needed to fulfill job responsibilities.
