Responsible to the Director of Behavioral health for clinical patient care, teaching, and assigned administrative duties. Specifically, the employee is responsible for provide evidence-based consultation, assessments and brief interventions within family medicine primary care practices serving populations across the lifespan; functions as a member of the clinical team through collaboration, providing consultation, assessment, treatment and behavioral health education; coordinating, supervising, and maintaining appropriate documentation for family medicine residents' community medicine educational requirements; evaluating residents and medical students performances in communication skis, systems-based practice, and community medicine and make recommendations as is appropriate; serving as a liaison between the residency program and the various organizations within the community; and performing other related duties as assignment. To provide collaboration with and a conduit to Behavioral Health and support an integrated model of health care; responsible for broadening the physicians' knowledge and skills to include patients' mental, social, and economic needs in assessment and treatment plans.
