This position reports to Manager of the Social Work and Care Coordination Department and works closely with the Nurse Care Coordinator and inpatient team to arrange post discharge appointments, ensure patients have primary care providers, transmit clinical information to following providers, and obtain outside medical records as need by the clinical team.RESPONSIBILITIES:Confirm patient's primary care provider or assist in establishing one, if needed. Notify primary care providers of patient admission, discharge and patient status updates. Arrange follow-up appointments post-discharge. Review follow-up appointments with patients/families to ensure feasibility and adherence. Assist in transmitting clinical information to providers conducting follow-up care post-discharge. Assist in obtaining outside records required by medical teams. Participate in practicum instruction of social work assistant students; Assist patients and their families with information regarding referrals as part of discharge planPerform other duties as requiredThis position provides opportunities to work in a fast-paced, challenging, diverse and rewarding environment. For detailed information on Benefits for this position, click here.REQUIREMENTS:Bachelor of Social Work or a Bachelor's Degree in the behavioral sciences AND two years experience as a Social Work Assistant I or equivalent.ORan Associate of Arts Degree in the behavioral sciences AND four years of work experience as a Social Work Assistant I or equivalent.Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.DESIRED:Experience in a fast paced clinic or hospital environmentFamiliarity with medical terminology. Strong organizational skills with attention to detailFamiliarity with electronic medical record and Excel spreadsheets
