In the Bridges Program/ Young Adult Transitional Unit, social workers provide support to patients and families with a high incidence of economic need, social and medical complexity.
This Clinical Social Worker will be responsible for:
Providing social work support to patients and families with a high incidence of economic need, social and medical complexity, and being an essential part of the interdisciplinary care for these patients and families.
Comprehensive psychosocial assessments, crisis intervention, coordination of care, linkages to community services, psycho-education on multiple issues, and management of family issues including vulnerable patient population protective/legal concerns and domestic violence.
Assessment of transition readiness, guardianship needs and care coordination with adult providers and systems.
Consultation and case collaboration to inpatient services treating young adults that are not housed on the Young Adult Unit.
Being a part of program development, teaching, and participation in initiatives aimed at improving patient outcomes.
To qualify, you must have:
Master's degree in Social Work, LCSW or LICSW in the state of MA.
Experience in a medical environment is preferred.
Flexibility and ability to work in fast-paced, dynamic environment.
Spanish language skills a plus.
This is a 20 hour/week position that may require some Saturday coverage.
