The Addiction Medicine consult service seeks a Licensed Clinical Social Worker (LCSW) to work as part of a multidisciplinary care team caring for patients with substance use disorder and providing counseling services and care coordination (including insurance prior authorizations and discharge transition planning). The position may involve collection and maintenance of data for reporting. A successful applicant will have excellent communication and organizational skills and will perform integrated behavioral health care case management services via coordinated care with providers and staff.
Must have experience working with patients with mental health diagnosis and/or substance use disorder, including those with chronic medical and mental health conditions. Must be able to provide individual, family and group therapy. Basic computer skills, including Microsoft office, excel. Must be able to independently navigate electronic health record.

Master's degree plus LCSW and three or more years of social work and/or mental health assessment experience highly preferred. Experience and passion for working with patients with substance use disorder preferred.
